GLP-1 Hair Loss: The Real Cause and How to Stop It

The Real Cause: Telogen Effluvium
About 3-7% of GLP-1 users report noticeable hair shedding. What\'s actually happening is telogen effluvium — a temporary increase in shed hair triggered by physiological stress, including rapid weight loss. The same shedding pattern occurs after bariatric surgery, postpartum, severe illness, or any rapid loss of 15+ lbs.
Mechanism: rapid weight loss is a metabolic stress signal. About 5-10% of your hair follicles prematurely transition from the active growth (anagen) phase into the resting (telogen) phase. About 8-16 weeks later, those resting hairs fall out — looking like a sudden increase in shedding.
Why Most People Get It Wrong
Trial data show GLP-1 hair loss rates are similar to placebo or only modestly elevated. The pattern strongly suggests the cause is the weight loss itself, not the drug. Evidence:
- Users who lose weight slowly on GLP-1 have less shedding
- The same shedding occurs in non-GLP-1 rapid weight loss
- Hair regrows once weight stabilizes (regardless of whether you continue GLP-1)
How to Prevent GLP-1 Hair Loss
- Hit protein targets aggressively. Aim for 0.7-1.0g per lb of goal body weight. Most GLP-1 users dramatically under-eat protein because appetite is suppressed. Use protein shakes if needed.
- Don\'t pursue extreme deficits. A 500-750 calorie/day deficit is plenty. Larger deficits accelerate hair shedding without faster fat loss.
- Slow titration. Stay at lower doses longer if you\'re comfortable; don\'t rush to maximum.
- Address nutrient deficiencies. Test ferritin (iron), vitamin D, B12, and zinc. Replace any low values.
- Resistance train 2-3x weekly. Preserves lean mass and reduces metabolic stress signal.

- Increase protein immediately to 1.0g per lb
- Test ferritin, vitamin D, zinc — replace deficiencies
- Consider topical minoxidil 5% (modest evidence for telogen effluvium speed-up)
- Reduce caloric deficit if it\'s aggressive
- Be patient: regrowth starts 3-6 months after stabilizing
