Overview
Rybelsus is semaglutide co-formulated with the absorption enhancer SNAC (sodium N-(8-[2-hydroxybenzoyl]amino)caprylate). SNAC temporarily disrupts the gastric mucosal barrier, allowing semaglutide — normally too large to absorb orally — to enter the bloodstream from the stomach. It must be taken on an empty stomach with no more than 4 oz of water, and you must wait 30 minutes before eating.
How Rybelsus Works
Same molecular mechanism as injectable semaglutide once absorbed: GLP-1 receptor agonism reducing appetite, slowing gastric emptying, increasing insulin, reducing glucagon. Bioavailability of oral semaglutide is only about 1%, so doses are much higher (3-14 mg daily) than injectable equivalents.
Dosing & Schedule
Rybelsus dose escalation:
- Month 1: 3 mg daily (starter dose, not therapeutic)
- Month 2: 7 mg daily (lowest therapeutic)
- Month 3+: 14 mg daily (maximum) if A1c not at target

Side Effects
Similar to injectable semaglutide but with somewhat higher rates of GI effects (nausea up to 20% at 14 mg) due to direct exposure of the stomach lining.
